Accident Report Form

This form is accepted throughout the Commonwealth of Massachusetts.  Copies can be made of this form, there is no need to complete three separate forms.  After completing the form, follow the instructions located in the lower right corner of the form.  If the accident occured in Canton, please drop a copy off at Canton Police Headquarters located at 1492 Washington Street or mail a copy,  Attn: Accident Report Officer.
